Navigating Hospital Admissions

At Advanced Medical Home Care, it is our goal to keep hospital admissions at a minimum for all of our patients. However, emergencies can happen and it is important to be prepared. In this blog, we are going to provide a list of some important details to keep track of regarding your hospital stay and your recovery.

Upon arrival to the hospital:

  • Inform hospital staff if you have a Power of Attorney
  • Provide a copy of your Advance Directives/Living Will if there is not one on file
  • Update your DNR preferences
  • Inform hospital staff if there is a specific person who can receive information about your hospital stay

During your hospital stay:

  • Stay informed about your plan of care
  • Ask for clarification if something is confusing
  • Inform your nurse about your mobility at home
  • Inform your nurse about your nutrition at home
  • Inform your nurse if you are having trouble sleeping
  • Ask about any medications you are provided
  • Inform nurse or social worker that you were receiving home health care services and give them the name of the company

Prior to discharge:

  • Check to make sure your home health services were arranged
  • Ask about any new medications that were prescribed to you and where to pick them up
  • Ask any remaining questions about information that may be unclear
  • Be sure to take home your After Visit Summary with your new medications listed
